Acknowledgments: By applying for consideration as a chaplain to the Soldiers of Christ Chaplaincy Program, I acknowledge my understanding and agreement that the selection of chaplaincy is at the sole discretion of Soldiers of Christ Ministries, based on its assessment of the overall qualifications and interview proceedings of chaplaincy applicants and with the Soldiers of Christ Chaplaincy Program ministry requirements.
Chaplain Expectations: I understand on signing this application that I obligate myself to share only the foundational Gospel of Jesus Christ. I understand that I am not to advocate any doctrines other than Jesus Christ as Lord and Savior and that I am not to promote my own church or denomination. I acknowledge that the Lord Jesus Christ is my personal Savior and that I am a personal ambassador of Him. I understand that I must conduct myself at all times in a manner that is consistent with Soldiers of Christ Ministries expectations as outlined in the organization's Statement of Faith and as detailed in Scripture within the Holy Bible. I further understand that I am committing myself to adhere to and uphold to the Scriptural teachings of Jesus Christ and to the principles taught within the Holy Bible. I understand and will adhere to and uphold the Soldiers of Christ Ministries Statement of Faith. I understand that if my conduct is determined by Soldiers of Christ Ministries to be inconsistent with its purpose, the result will be corrective action up to and including termination from chaplaincy and/or volunteer status. I agree that information obtained during the course of my time with Soldiers of Christ Ministries (including but not limited to, the personal information and stories of those to whom I minister) is to be considered confidential information, and I agree to view, handle, and hold all confidential information in trust and confidence and will not disclose of such confidential information to unauthorized use or users or publications, and agree to not directly or indirectly reveal, report, publish, transfer, or otherwise use any of the confidential information except as specifically authorized by Soldiers of Christ Ministries.
Assumption of Risk and Releases: I understand that while volunteering and/or working with Soldiers of Christ Ministries, I may be exposed to dangerous or hazardous situations and environments and may engage in hazardous work involving risk of illness, physical injury, property damage, or even death, resulting from physical labor and exposure to environmental and equipment hazards. I hereby assume all risks associated with performance of these activities and release and forever discharge Soldiers of Christ Ministries from any and all liability from the items specified above to include provisions of food, lodging, transportation, equipment, or activities in connection with volunteering and/or working with Soldiers of Christ Ministries. I understand that Soldiers of Christ Ministries is not responsible for the safety or security of my personal property and therefore, do hereby release Soldiers of Christ Ministries from all liability of theft, damage, or destruction of my personal property.

Consent to Release Information: I understand that in consideration of my application to become a chaplain, or my continued service as a chaplain with Soldiers of Christ Ministries, an investigation may be conducted of my past employment and other activities. I hereby authorize past employers, personal references, and all other associations to answer all questions asked concerning my previous employment, character, ability, government, professional and/or civilian service, civil and/or criminal history, driving histories, and educational background. I release all such persons from any liabilities or damages having resulted from releasing such information. I hereby authorize Soldiers of Christ Ministries and their agents to conduct such an investigation and release all associations and their representatives from all liability or responsibility for this investigation.

Chaplain Certification Commitments
Cost for certification as a Chaplain with Soldiers of Christ Ministries is $378.00 per year or can be payed in installments of $31.50 per month for Individual Chaplain accounts. For married couples seeking chaplaincy training together there is a joint Family Account cost of $474.00 per year; this can also be payed in installments of $39.50 per month. I agree to pay $378.00 in full for my yearly chaplain certification.I agree to pay monthly installments of $31.50 a month for my chaplain certification. This will total $378.00 for my yearly commitment.I agree to pay $474.00 in full for my yearly chaplain certification under a joint married family account.I agree to pay monthly installments of $39.50 a month for my chaplain certification under a joint married family account. This will total $474.00 for my yearly commitment.I am applying for a Chaplain Scholarship. This scholarship will apply for one year and is dependent upon approval of my application and availability of scholarship funds.
I agree to uphold my commitment of payment for my Chaplain Certification through Soldiers of Christ Ministries. This is a yearly cost and I agree to pay either in full yearly or to have payments distributed monthly depending on my selection above. If I select monthly installments I acknowledge that this will be debited from my preferred banking account on the same day of every month, beginning with the first day I create my chaplain account on the Soldiers of Christ Ministries website. If I selected to pay for my certification in full I understand that this will automatically renew every year unless I decide to cancel beforehand. Monthly installments will also automatically renew every year unless I decide to cancel beforehand. The Soldiers of Christ Ministries website will process my payments via Stripe's automated payment platform.*

If approved for a scholarship, a debit or credit card is still required to create an online chaplain account. A scholarship code will be given at the creation of my account and applied for a yearly discount. I understand that I must request to renew my scholarship every year before the anniversary of my initial start date of my chaplain online account. Failure to request and receive approval beforehand will result in my debit/ credit card to be charged for the next year's chaplain certification. If a scholarship is no longer to be applied during my time as a chaplain with Soldiers of Christ Ministries, I agree to uphold my yearly financial commitment as outlined above.*
