We take photos and use other media of the children, young people and families we support to help raise funds and awareness about our work. Please use this form to let us know if you are happy for us to take and use photos, audio and/or videos of you and/or your child / young person, of whom you confirm you are the parent or guardian of.
I give permission for photos, film and/or audio of me and/or my child / young person to be used for Spread a Smile’s communications and marketing in order to publicise Spread a Smile’s activities. This includes but may not be limited to Spread a Smile publications and literature, external publications, newspapers, magazines and other print media, on television, radio and electronic/digital media including the internet and social media. I understand that they may be published on Spread a Smile’s website, other publications and in Spread a Smile’s newsletters. I also understand that supporters of the charity and other parties might then re-post these stories to further media channels, including other websites or social media. My and my child/young person’s images, audio and/or video may be used by third parties that are associates or partners of Spread a Smile to further promote Spread a Smile’s activities.

Spread a Smile is the controller and responsible for the personal data comprising the photos, film and/or audio and your contact details and any related personal data of you, and your child/young person. Spread a Smile will:

(a) ensure that we comply with the requirements of all legislation and regulatory requirements in force from time to time relating to the use of the photos, film and/or audio and your contact details and any related personal data of you and your child/young person; and
(b) process the photos, film and/or audio and your contact details and any related personal data of you and your child/young person in accordance with our privacy policy which can be accessed via our website here or sent to you via email.
This letter shall be governed by and construed in accordance with the law of England and Wales and courts of England & Wales shall have exclusive jurisdiction to settle any dispute hereunder.
