This is a statement in which you are informed of some potential risks involved in participating in scuba diving in a Treasure Hunt, and of the conduct required of you during this event. Your signature on this statement is required in order to participate in the Treasure Hunt being offered by Torpedo Rays Scuba Adventures, located in Hubbards, Nova Scotia. Read and discuss this statement prior to signing it. If you are a minor, you must have this statement signed by a parent.
Diving is an exciting and demanding activity. When performed correctly, applying correct techniques, it is very safe. When established safety procedures are not followed, however, there are dangers.
To scuba dive safely, you must not be extremely overweight or out of shape. Diving can be strenuous under certain conditions. Your respiratory and circulatory systems must be in good health. All body air spaces must be normal and healthy. A person with heart trouble, a current cold or congestion, epilepsy, asthma, or a severe medical problem should not dive. If taking medication, consult your doctor and Torpedo Rays Scuba Adventures before participating in this event. You must complete this statement before enrolling in this event.

Affirmation and Liability Release

I understand and agree that Torpedo Rays Scuba Adventures may not be held liable in any way for any occurrence in connection with this Treasure Hunt event which may result in injury, death, or other damages to me or my family. In consideration of being allowed to enroll in this event, I hereby personally assume all risks in connection with the program, for any harm, injury or damage which may occur while I am participating in the program, including without limitation, all risks connected therewith, whether foreseen or unforeseen, whether or not occurring as a result of the negligence of Torpedo Rays Scuba Adventures.
I further state that I am of lawful age and legally competent to sign this affirmation and release, or that I have acquired the written consent of my parents or guardians: that I understand the terms herein are contractual and not a mere recital; and that I have signed this document of my own free act. IT IS MY INTENTION BY THIS INSTRUMENT TO EXEMPT AND RELEASE TORPEDO RAYS SCUBA ADVENTURES FROM ALL LIABILITY WHATSOEVER FOR PERSONAL INJURY, PROPERTY DAMAGE OR WRONGFUL DEATH ARISING OUT OF THE NEGLIGENCE OF TORPEDO RAYS SCUBA ADVENTURES.
I ACKNOWLEDGE THAT I HAVE READ, AND UMNDERSTAND THE PROCEEDING TREASURE HUNT STATEMENT BEFORE SIGNING IT. I VERIFY THAT I AM PHYSICALLY FIT, NOT UNDER THE INFLUENCE OF ALCOHOL POR DRUGS AT THIS TIME, AND SUFFICIENTLY QUALIFIED TO PARTICIPATE IN THIS EVENT.
