    Distinctability services’ responsibilities:

    • To treat you with courtesy, dignity, and respect
    • To support your right to exercise choice and control over the support received
    • To respect your right to access an advocate
    • To provide you with supports and services free from violence, abuse, neglect or exploitation or discrimination
    • To discuss with you any concerns you may have including any complaints and feedback
    • To respond to any inquiries in a timely manner
    • To provide you with service in accordance to your NDIS plan
    • To pay your providers in a timely manner and as per the Distinctability Services Plan Manager - Business Terms
    • To claim only the funding allocated to us
    • To collect, maintain and store your information in accordance with Distinctability Privacy Policy (accessible via our website www.distinctability.org.au)

     

    The Participant’s responsibilities:

    • To treat the staff at Distinctability Services with courtesy and respect
    • To let us know if you have any concerns or complaints by calling or emailing us. Complaints can be submitted via email directly to complaints@distinctability.org.au
    • To consent Distinctability Services to collect, use and retain information related to your NDIS plan and support received
    • To provide us with a copy of your NDIS plan
    • To log in onto the NDIS MyPlace portal and share your plan with the Plan Manager
    • To inform us if there are any changes to your personal details
    • To provide us with notice if you make any changes with your providers or services
    • To give us notice if you are undergoing a review with your NDIA planner
    • To approve all invoices in writing through email. See Approval of Invoices section later in this document.

     

    Continuation and cancellation of the service agreement:
     

    • This agreement is ongoing and will cover the current and any future NDIS plan that the participant will have.
    • The parties agree to continue this agreement until either party gives 14 days’ notice in writing.
    • The service agreement can be terminated without notice in the event that either party seriously breach the terms of this agreement.

    Service booking
     

    Distinctability Services will create a service booking on your behalf via the NDIS MyPlace Portal so we can manage payments to your nominated providers for services delivered. This booking will confirm:

    • The service start and end date
    • Distinctability Services is acting as a Plan Manager
