  • GSLPS Insurance Fee/Mobile Device Parental Consent Agreement

    Students will now be issued a mobile device for the year to be able to use it at home. There is a fee of $30.00 to cover the insurance cost on the device. The fee covers accidental breakage or loss of the device. The $30.00 insurance fee is non-refundable. If no fee is paid and breakage or loss occur, a fee of $300.00 will be applied per Chromebook, or iPad.

  • Student Responsibilities

    • Students are expected to use the device appropriately for educational purposes.

    • Students may not install software or download apps without the direct permission from the instructor or the Technology Department.

    • Students are responsible for saving or backing up their own work or files.

    • Students must follow all guidelines in the Technology Acceptable Use Contract.


    Parent Responsibilities

    • Parents must sign this agreement form.

    • Parents should help to ensure that only the student is using the device and that it is being used appropriately.

    • Any damage to the device must be reported immediately to the instructor or the Technology Department.

    • Ensure that all guidelines in the technology Acceptable Use Contract that was signed at the beginning of the year are followed.

    • Please remember these mobile devices are the property of the school district. Therfore, if your student leaves the district for any reason, the mobile device must be returned to the district promptly.
