  • Health Certificate Waiver Of Responsibility

  • Regulations for pets traveling outside of Oregon are governed by the destination States or Foreign Countries. Those regulations and requirements may change at any time. The requirements and forms may be complex and may not be readily available to us. We recommend pet owners obtain written documentation for the requirements needed from the embassy or consulate of the country of destination and any other countries the pet may need to enter. Ideally, this documentation will reference the date and the person's name that has provided the information.

It is the client's responsibility to determine the paperwork, tests, vaccines, and identifying information required for your pet to be admitted to its destination - foreign or domestic.

Additional costs may occur and may include six months of quarantine for the pet if detailed requirements have not been completed or if forms are not properly filled out and endorsed. The costs associated with any additional treatment and/or quarantine, and/or the cost associated with the refusal for the pet to enter the country of destination is the responsibility of the pet owner.

We take our guidance from the United States Department of Agriculture Animal and Plant Health Inspection Service (USDA APHIS). However, it is ultimately the pet owner's responsibility to ensure all requirements have been completed and in a timely manner and to read through the completed documents to check for any errors/typos/misspelled information. If an error is discovered and the pet is declined entry into the country of destination it is the pet owner's responsibility to cover the costs of a new health certificate. Any errors discovered while the pet is still at Brooklyn Yard Veterinary Hospital will be corrected at no additional cost.

We make no claim of warranty, suitability or acceptability for the international health certificates beyond our endorsement on the forms provided by the owner and/or the USDA APHIS office. We do not warranty in any way that the form used is the correct form required by the nation or nations at the time of endorsement or at the time of travel. We do not warranty in any way that the pet will not be required to be quarantined.

I understand that regulations for the travel of my pet are regulated by the country or countries my pet may visit. I understand that I am ultimately responsible for ensuring that the requirements are completed in a timely manner and the proper forms are created. I agree to hold Brooklyn Yard Veterinary Hospital harmless for any errors or omissions or expenses related to requirements and documentation for my pet's travel.

  • We will work diligently to ensure your pet meets the veterinary requirements for travel, however, due to the extensive preparation needed for international or Hawaii health certificates, we expect the following from our clients:

    1. Thorough review and understanding of ALL requirements for destination. We can only assist with the health certificate and veterinary documents needed for travel, all other requirements are the pet owner's responsibility.
    2. Complete previous medical records - we cannot answer specific questions about travel requirements until all medical records are reviewed. We are happy to contact other clinics to obtain records!
    3. Reliable communication. After review of the destination requirements and medical records, we will reach out via email with next steps which may include time-sensitive vaccinations and labratory tests.
If we do not receive a response after 7 days to any inquiry, we will make a final attempt via phone call and email. If no response, we reserve the right to discontinue services.
