    • Grant Impact Report Section #1 - Formal thank-you letter 
    • Your words of gratitude may be forwarded to our Board and donors, or used in publications like The Harvest or our newsletters. Please take the time to express yourself thoughtfully and in detail, even if you have already thanked the Foundation by phone or email.

    • Grant Impact Report Section #2 - Grant activities 
    • Please briefly recap what your organization proposed to do with the grant and how the project was implemented. One or two paragraphs are sufficient. In this section, please explain:

      1. Any special, positive outcomes from this grant that were unforeseen
      2. How many people have been, or will be, significantly impacted by the project this grant supported? Who will benefit?

       

      If you’d like, you may include this section in your thank-you letter.

    • Grant Impact Report Section #3 - Testimonials and stories (required) 
    • Help us show the difference this project has made to your Catholic organization and/or community. We are looking for participant quotes, testimonials, or stories of transformation—any good news that is a direct result of your project.

       

      Testimonials, quotes, and stories are of critical importance to the Catholic Foundation of Eastern Montana. Your grant impact report will be considered incomplete without them, which factors into future grant award decisions.

    • Grant Impact Report Section #4 - Proof of proper use of funds (required) 
    • The Foundation must receive evidence that the grant has been used for the intended purpose within 12 months of receiving the awarded funds.

       

      There are two parts to this: (1) the Grant Expense Record (which lists all grant-related expenses), and (2) invoices/receipts that prove the money was spent. Without both, your report will be considered incomplete.

      1. Complete your “Grant Expense Record” online (https://form.jotform.com/222345904970054). This form shows how grant funds, and other funds for your project, have been used. Make sure that you list the ways that all your grant funding has been spent (you should not have any extra funds from the Foundation unaccounted for).
      2. Then, upload documents—such as invoices or receipts—that prove that the money was actually spent.

    • Grant Impact Report Section #5 - Proof of grant promotion (required) 
    • Please upload proof that you have promoted your grant from the Catholic Foundation of Eastern Montana, using our full name and logo (which will be emailed to you). Examples may include:

      • A scanned copy of your bulletin, which contains an announcement that you received a grant from the Catholic Foundation of Eastern Montana (template announcements will be emailed to you)
      • An article you submitted to The Harvest mentioning your grant from the Catholic Foundation of Eastern Montana (please make sure to use the organization's full name as written—we can help you write or edit)
      • Screenshots of Facebook or other social media posts stating that you received a grant from the Catholic Foundation of Eastern Monana (we can create these for you as well, if desired)
      • A PDF or scan of your organization's newsletter mentioning your grant (we can write an article for your newsletter, however long you want it to be—just contact the Foundation for help)
      • Many other options. Our staff are happy to help you find and create the right way to let your supporters know about your grant.

    • Grant Impact Report Section #6 - Photos of your project (required) 
    • Please upload photos of your grant project, preferably before and after (if applicable). Think to yourself, what kinds of pictures would look good in The Harvest to help people learn about the Foundation?

       

      Include smiling people for bonus points!
