By submitting this request form I am agreeing to the following:
- A deposit of $50 or $100 may be required to schedule
- Participants will not receive their certification cards until full payment is complete
- All returned checks are subject to a $35 fee
- Groups, Businesses, and Organizations who cancel within forty-eight (48) hours of the start time of their scheduled course are responsible for a $50 cancellation fee
- Groups, Businesses, and Organizations who cancel within twenty-four (24) hours of the start time of their scheduled course are responsible for a $100 cancellation fee
