What is Social Prescribing and who is it for?
The Social Prescribing Service is part of a new suite of programmes being offered under the Sláintecare Healthy Communities Programme in Clondalkin and Tallaght.
The Social Prescribing Service is a free service that supports people to connect with their community and improve health and wellbeing through engaging with local services, activities or groups. These may include mental health and wellbeing programmes and supports, physical activity groups, volunteering opportunities and social/community groups. Social Prescribing is a non-clinical service and participation is voluntary.
Anyone living in the Clondalkin or Tallaght Healthy Communities catchment area can be referred to the Social Prescribing Service if it represents the most appropriate tool for addressing their needs.
Social Prescribing can be particularly beneficial for:
- People who are lonely, isolated or disconnected
- People who experience mild-moderate anxiety/depression
- People living with one or more chronic health condition(s)
- People who have been recently bereaved
- Members of marginalised communities or cohorts
What are the benefits to people accessing the service?
One of the main advantages of Social Prescribing is the opportunity for people to access non-clinical supports and activities within the community and voluntary sector, which can lead to positive outcomes associated with health and wellbeing, such as reduced isolation and increased independence.
Referral Criteria:
- Adults over the age of 18
- Living in the Tallaght or Clondalkin Healthy Community Catchment area
- With the ability to actively engage with the Social Prescribing Service
Social Prescribing is not suitable for:
- People who are currently in crisis - Please refer to the appropriate crisis support service.
- People who are unable to leave their home and return safely.
- People who are presenting with severe mental health condition(s), which have not yet been addressed.
