    Chester FC Community Trust is delighted you wish to join the Chester FC Ball Crew!

    You will play an important role on matchdays and this consent form includes some useful information for you and your parent or guardian.

    About the role

    • This is a voluntary position and we will ask you to confirm which fixtures you can attend. 
    • Your primary role is to return the ball to the players or pitch during matches so that the game can continue.
    • As a member of the ball crew, you're representing Chester FC so it's important you behave responsibily and are a good role model for the club.
    • The matchday coordinator will tell you everything you need to know, but if you have any questions or are not sure about something, please ask.
    • We ask you to wear sensible clothing and will also provide some kit to make sure you look the part and to help keep you warm and dry.
    • We will provide some training, make sure you know who is responsible for you at a  match and give you refreshments.
    • We ask you to be professional, perform the role to the best of your abillity and, most of all, have fun!

    What you need to do

    • The main purpose of this role is to act quickly and fairly in returning the ball into play during any game you are invited to come to. You must hand it or throw it gently back to the player who wants it.
    • You may need to listen to the referee, assistant referee or stewards if they are trying to help you to do your job
    • You need to listen to instructions given to you by the matchday cordinator and do what they ask as long as you feel comfortable doing it.
    • You should avoid distracting the players before, during, or after the game. They have a match to focus on!
    • You will be required to sit quietly, ready to fetch the ball for the duration of the match.
    • On matchdays, we ask you to arrive 45 minutes before kick-off and report to the main entrance.
    • At the end of the game, you should report to the matchday coordinator who will wait with you until your parent or guardian has collected you.
    • If you see or hear anything that you are worried about, please talk to the matchday coordinator or another member of staff like a steward.

    In order for your child to be part of the Chester FC Ball Crew, we need you to complete this form and provide consent as their parent or guardian.

    By completing this form you are confirming that you have legal parental responsibility for this child.

    By consenting you are stating that your child is in good health and that they are not participating contrary to medical advice.

    In the unlikely event of an accident occurring, you are providing permission for a designated representative of the club to authorise emergency medical treatment, including the use of anaesthetic if deemed necessary. We will take all reasonable steps to contact you in such circumstances.
