The Works High School Group Registration

The Works High School Group meets Tuesdays from 7:30-9pm. We focus on the Works of Mercy, learning about the corporal and spiritual works of mercy and then perfom those works throughout Fort Collins (ie: Feed the hungry: handing out burgers to homelesss in Fort Collins We will also have a fall retreat in Estes Park. We meet on Tuesday nights 7:30-9 pm. High School students meet two weeks of the month in the youth room, one week for adoration at St. John XXIII, and one week of the month for small group discussion at Panera in Front Range Village.

Please complete one form per High School child.

If your child is in need of any of the Sacraments, we are ready to help walk with your HS student as they prepare to receive Sacraments! When making payment, please be sure to select a "Sacrament Preparation" ticket in addition to the Faith Formation registration ticket.

 

Save the date of Wednesday, September 21, 7pm-8pm, for meetings for all MS & HS students & and their parents preparing to receive sacraments. Char Selken, our volunteer catechist working with MS & HS students preparing to receive the Sacraments, will be in touch with more details and to set up a meeting if she hasn't previously met you and your family.

As the parent/guardian of,I give permission for my high school student to be contacted in the following manner(s) during the faith formation year (Fall 2022-Summer 2023 Please make sure your student is also aware of your preferences below. Please check all that apply:

Parish prepares materials that support and promote the Catholic Faith and Church ministry and programming, and which generally reports on both Church and non-Church events and activities. These materials include, but are not limited to Parish bulletins/periodicals and its website(s The materials often incorporate images, video or other media featuring individuals. Additionally, so as to grow Catholic ministry and spread the teachings of Christ, the Parish also works with other Catholic entities (including, but not limited to, the Archdiocese of Denver) in the creation of spiritual, educational, promotional and development materials.

By signing below, You are agreeing that the Parish may use Your Likeness (as defined below) per the terms below. If signing on behalf of your minor child, You and Yours refers to your minor child.

1) Definitions. For purposes of this Release, the following definitions apply:

a) The "Parish" means the above named parish;

b) the person whose Likeness is sought is referred to as the "Individual";

c) the terms "I", "You", "Your" means parent/guardian. "My" means student. 

 

d) "Likeness" means all parts of the Individual's identity, including, but not limited to, the Individual's name, image, photograph, signature, voice, quotations, recordings, and other aspects of the Individual's right of publicity. 

i) only the Individual when the Individual is 18 or older;

OR

ii) both the Individual and the Individual's parent/legal guardian when the Individual is under the age of 18. 

2) Release. 

a) You hereby consent to, agree, and otherwise grant permission to, the Parish and its agents:

i)taking the Individual's photograph, recording the Individual's image, and/or making video of the Individual, inclusive of audio, all either digitally or by a non-digital device for the purposes herein;

ii)using the Individual's Likeness, inclusive of video with or without audio recordings, for any lawful purpose not inconsistent with the Parish's mission and purpose, including but not limited to use in Parish publications, or on the Parish website(s), or in any medium now kno.vn or later developed; and

iii)using the Individual's Likeness as a reference and colorizing, cropping, editing, manipulating, miniaturizing, distorting, caricaturing, animating or otherwise changing the Individual's Likeness.

b) You hereby grant and convey unto the Parish all rights, title and interest in and to all photographs/images/recordings of the Individual made by the Parish or its agents of the Individual, including, but not limited to, any royalties, proceeds or other benefits derived from such photographs/images/recordings. For clarity, you agree that the parish will own any materials in which it uses the Individual's Likeness. 

c) You acknowledge and agree that you will not be paid for signing this Release or allowing the Individual's Likeness to be used. You will, however, have the change to be part of promoting the Parish and the Catholi Faith. You further agree that you will not have any approval rights of how the Individual's Likeness is used, and that the permission graned under this Release is not revocable. 

3) General Terms. You will not sue, dispute, challenge, raise objections to or institute proceedings against the Parish for using the Individual's Likeness in anv manner and You herebv waive all legal and equitable rights You may have as well as all claims, demands, damages and expenses arising directly or indirectly from the same. You agree that You are under no obligation to sign this Release, but do so voluntarily, without duress, fully understanding Your rights and having had the chance to consult with an attorney.

4) Minor Acknowledgment. By signing this Agreement without a parent or legal guardian's signature, You, under penalty of fraud, represent that You are at least 18 years of age. If signing as the parent or guardian of a minor Individual, You represent that You are the legal parent or guardian of the minor Individual.
