Upon entering events sponsored by Waskaganish First Nation, I/we agree to abide by the rules set by the organizers, coaches, instructors & monitors. I/we understand and appreciate the participation or observation of the sport constitutes a risk to me/us of serious injury, including permanent paralysis or death. I/we voluntarily and knowingly recognize, accept, and assume the risk and release Waskaganish First Nation, and its affiliates, Rupert River Sports Complex, and/or its representatives, their sponsors, event organizers and instructors from any liability therefore.
CONSENT TO TREAT
Authorization for necessary medical treatment during the absence of parent or legal guardian. I acknowledge that necessary medical care may be administered to my child during my absence or in the event I cannot be reached immediately. Any designated physician is authorized to perform treatment.
