  • 1. Card on File Policy for Appointment

    By providing a signature below, I understand:

    To schedule an appointment(s), an authorized debit or credit card must be placed on file. Placing a card on file will hold any appointment(s) made. Failure to provide an authorized debit or credit card will result in an automatic cancellation of my appointment(s).

  • Cancellation & No Show Policy

  • 1. Cancellation/No Show Policy for Appointment 

    By providing a signature below, I understand:

    If my scheduled appointment is not cancelled at least 24 hours in advance, a 50% charge of the appointment will be forfeited. No show appointments result in a 100% charge of my appointment(s). If I fail to show to my appointment(s) more than once, a 100% charge of the appointment(s) will be forfeited and I will be unable to schedule any services provided by The Cozmo Haus until further notice.

    *I understand that there are times when you must miss an appointment due to emergencies or obligations, so please contact me prior to make adjustments. 


    2. Early/Late Policy for Appointment

    By providing a signature below, I understand:

    If I arrive early I will need to wait until my scheduled appointment(s) time to be serviced.

    If I am late 15 minutes pass my appointment(s) scheduled time, my appointment(s) will be cancelled and a 50% charge of the appointment will be forfeited. If I am consecutively late more than two (2) times, a 100% charge of my appointment(s) will be forfeited and I will be unable to schedule any services provided by The Cozmo Haus until further notice.

    *I understand that delays can happen, however, my schedule must run on time.

  • No Additional Guest(s) Policy

  • 1. No Additional Guest(s) Policy for Appointment

    By providing a signature below, I understand:

    Due to limited space within the studio, The Cozmo Haus prohibits any additional guest(s) and/or children from attending scheduled appointments. First failure to comply will result in rescheduling/cancelling my appointment(s). If I bring additional guest(s) and/or children to the studio after being notified of this policy, again, a 100% charge of the appointment(s) will be forfeited and I will be unable to schedule any services provided by The Cozmo Haus until further notice.

  • After Hours/V.I.P. After Hours Services Policy

  • 1. After Hours Service Policy for Appointment

    By providing a signature below, I understand:

    After hours services are available for lash extensions and brow services. I may schedule Monday or Saturday, from 5PM to 7PM ET, with the permission of The Cozmo Haus. 

    2. V.I.P. After Hours Service Policy for Appointment

    By providing a signature below, I understand:

    Once registered to the V.I.P. Club, an amount of $50 will be withdrawn on the 30th or 31st of every month from the designated card on file. This allows me to schedule unlimited after hours appointments for the month. I may cancel at any time with at least 24 hours prior to my next billing date.

     

  • Refill & Refund Policy

  • 1. Refill Service Policy for Appointment

    By providing a signature below, I understand:

    A refill service requires that I have at least 30-50% of my lash extensions to proceed with the appointment. If I do not qualify for this appointment, I will be rescheduled/cancelled and accommodated with the appropriate service. This may require scheduling to a different day if length of the appropriate appointment overlaps other appointments. If I schedule for a different day, my card on file will hold the rescheduled appointment. If I fail to comply with the requirements of this policy more than once, I will be unable to schedule any services provided by The Cozmo Haus until further notice.


    2. Refund Policy of Appointment

    By providing a signature below, I understand:

    I will be given 72 hours after my scheduled appointment(s) to notify The Cozmo Haus of any reasonable, unsatisfactory result of the service(s) provided and will be accommodated with no charge. If I fail to contact The Cozmo Haus within 72 hours, I will need to schedule a separate appointment. There are no refunds to services.
