Danny Hall Memorial Scholarship Application

Applicants for this scholarship are required to complete the form below and provide one letter of recommendation.
Winners are selected from the application pool once per quarter meaning that four scholarships are awarded each year. Applicant will continue to have chances to win without re-applying. Once an application is submitted, it will remain active in the drawing until one of the following occurs:
- Applicant’s name is chosen from the quarterly drawing
- The applicant graduates from high school
- Applicant wishes to withdraw their application from the drawing

Terms of agreement:
1. Scholarship recipient will have 1 week after notification to respond with confirmation and acceptance of their win. Failure to do so will result in forfeiture and a different recipient will be chosen.
2. The recipient must confirm acceptance by signing the terms of agreement.
3. Upon acceptance, the recipient will have 3 months to activate their contract and begin music lessons.
4. MDMEF will provide 12 months of music lessons valued up to $1620.
5. The recipient has 14 months from the time the first lesson starts to utilize the scholarship funds.
6. It is the student’s responsibility to coordinate their lessons with the approved school.
7. MDMEF will make payment directly to the school on a monthly basis upon receipt of invoice from the school.
