All UTEFR members must successfully complete a skills evaluation every year to volunteer with UTEFR/UTEFR Partnerships as event medical staff. A skills assessment is intended to ensure that all Active Responders are able to competently perform first aid up to the level of their training. Please use this form to book an appointment for a skills evaluation.

Note ALL Skills Evaluation sessions will take place in Sidney Smith Hall, Room 581.

The room is hard to find, please use the following directions:

1. Room 581 is in the basement of Sidney Smith Hall.

2. Enter the building from the main entrance on St. George Street.

3. Use the first set of stairs to your left as you enter the building, right beside the student commons.

4. Proceed down the stairs to the purple section of the basement (on your left after exiting the stairwell)

5. Room 581 will be down the hall.
