PLEASE READ BEFORE COMPLETING:

This document serves as an request only that the following person applies for the animal listed below to possibly be re-homed through/by Majestic Beast Rescue (MBR). Should your request be successful MBR will contact you for an Behavioural assessment of the below mentioned dog to be paid for by you based on area of residence a price will be given. Please note that MBR relies solely on public donations to keep our organisation going everyday thus we cannot offer this service for free as we use certified behaviourists for these assessments.

Should MBR decide to assist in rehoming based on the behavioural assessment of the below mentioned dog, sterilization, vaccinating and microchipping the dog before being placed into a new home will be compulsory if not already up to date. (Majestic will take responsibility in doing so)

Once MBR agrees to assist in finding the dog a suitable permanent home, any and all adoption requests are to be referred to MBR. info@majesticbeastrescue.org

I hereby request assistance in re-homing my animal. I further certify that I understand the terms of this Re-home Assistance Form. I hereby agree to care for the animal if request is approved, while Majestic Beast Rescue networks the animal for a new permanent home. Once a suitable home is found MBR will follow their adoption procedure.
