This document serves to confirm that the following person applies for the animal listed below to be re-homed by Majestic Beast Rescue (MBR) MBR will assist in rehoming the animal, sterilizing the animal & vaccinating the animal before placed into a new home. ONLY MBR is allowed to place the dog in a suitable, permanent home. Any and all adoption requests are to be referred to MBR. info@majesticbeastrescue.org

I further certify that I understand the terms of this Re-home Assistance Form. I hereby agree to care for the animal while Majestic Beast Rescue is networking the animal for a new permanent home. Once a suitable home is found MBR will follow their adoption procedure.
