  • Will the room/location the items are being delivered into be empty and ready for the new merchandise?*

  • Please verify that all DRIVEWAYS, WALK WAYS AND ENTRANCES (vehicles moved in advance is appreciated) are clear of hazards for the team in advance.(Pictures removed from the walls,boots/shoes out of the entrance way) For winter months, Snow and Ice(ice melt/sand distributed) are requested to be cleared prior to arrival in order to assist with the success of a scheduled delivery. Are the walkways safe to traverse upon arrival or are there hazards the team must be aware of(Use other with comments please) Pets to be secured.*

  • What Category is being delivered to you (Please select multiple categories if needed)*
  • Have you confirmed that your new Furniture will fit through your doorway into your home. Travel through your home interior, and fit into the desired location in your home? 90 degree turns must have enough room to allow for merchandise such as sofas to be moved vertically.*
  • With your new mattress purchase, are you expecting to receive new boxsprings on the scheduled delivery?*
  • Are you Getting Delivery of any wood merchandise. Example (coffee tables, bedroom furniture, dining room furniture, tv stand etc)*
  • All wood furniture comes in FLAT BOX/READY TO ASSEMBLE condition. Unless assembly service has been purchased or otherwise noted, your wood furniture will be delivered in a flat box condition. Furniture Galaxy will not assemble merchandise unless prior arrangements have been made.)*
  • Are you getting a delivery of a Headboard, Footboard, and Rails?*
  • All Beds come in FLAT BOX/READY TO ASSEMBLE condition. Unless assembly service has been purchased or otherwise noted, your bed furniture will be delivered in a flat box condition. Furniture Galaxy will not assemble merchandise unless prior arrangements have been made.*
  • Please note that Furniture Galaxy will NOT disconnect or reconnect existing or new appliances.

    All New Appliances will be brought into your home, placed in your desired location, and packing removed.
  • Are you getting any appliances removed for your home?*
  • Please note that Furniture Galaxy will NOT disconnect or reconnect existing or new appliances.

    Removal Service of Appliances can only be completed if existing appliances in home are uninstalled. Failure to have the appliance uninstalled in advance will result in the removal portion of the delivery not to be completed.
