I First Name Last Name understand that this form is a request to showcase designs in the JUNE 15 2024 Put on for My City Youth Fashion/Talent Show PART 6. I understand that if I am selected as a featured designer I will be provided a Look book to choose models from to showcase designs on the red carpet. I understand I am allowed to have my own models however they must register as a model through the website and follow the show process for all models. I understand that my designer spot is not secure with out my designer registration payment of $150. I understand my entry automatically places me in the Designer competition and there will be a cash prize & award for crowds voted best designer I understand that Tailor-Made by Tae LLC will promote Me as a designer on all social media platforms and it is expected that I the designer also promote my participation into this show.
