Because energy healing moves stuck energy in the auric field as well as on the actual physical body, we need to be aware of any medical and psychological issues you may have to ensure that this type of healing is right for you.  We would want to take safety precautions for any conditions that might be counter-indicated with energy healing.  Please answer all questions truthfully keeping in mind that this form is confidential and will only be shared with faculty and staff if precautions need to be taken in any healing situation.

1.  I understand that Spiritual Energy Healing is in no way a substitute for medical or psychological treatment.  I affirm that I have stated all my known medical and psychological conditions and answered all questions truthfully.  I am open and receptive to the process of transformation and am willing to do my part.  I will in no way hold Twin Flame Creations Inc. or any faculty, student or healer therein responsible for any side effects alleged to be caused by my healings or the information I will receive in class and take sole responsibility for my own well-being.  I will inform the school faculty of this school of any changes to my medical/psychological self in a timely manner regarding future healings.

2.  During your time at The Awakening Angels School For Healing, Enlightenment and Ascension Mastery, you will experience and witness healings taking place at many different levels.  In order for you and everyone else present to feel safe to express, process and release negative energies, we must agree that what happens during these classes including all personal discussions and healing work must stay confidential and within the school body.  This will provide a level of personal comfort and feelings of safety that will make it easier to allow healings to take place.

Therefore, I agree to uphold and follow the confidentiality clause set in place by Twin Flame Creations Inc. and I agree not to discuss the healings of others that take place during classroom or homework time outside of the school.  This also means that I will not post any private discussions or other school business also covered by this contract on Facebook, Instagram or any other social media.  If I should break this contract, I agree that I may be subject to immediate dismissal from this school.

3.  I am participating in a program that contains copyright material both written in the student workbook as well as original meditation material.  I understand and agree that all written, spoken or other materials including all original artwork and music which may be presented during class are the property of Twin Flame Creations Inc. and that any videotaping, photographing or copying by any means known either now or in the future of any portion of this material is prohibited by law and protected by copyright laws.  I further agree to not duplicate, disseminate, teach or freely share in any way the materials without written permission from Twin Flame Creations Inc.

4.  During classes, we may take pictures of you as an individual or as a group that we will post on our private Facebook page that is only seen by other students and faculty.  With your agreement, you will be added to this group after the Level 1 class in order to stay in touch with everyone.  We occasionally like to share group photos on our public website or other social media.  We agree however, to ask  you for your verbal or written permission before publishing any picture that you are in on any public forum.  We also agree to take down any pictures of you that you don't want shared with anyone.

Therefore, I agree to allow Twin Flame Creations Inc. to use my class group picture on their public website.  If I do not want this published, I agree to tell the staff before any group pictures are taken.  Any other pictures of me will be subject to my verbal or written permission.  I also agree to allow all pictures of me to be posted in a private, student only Facebook group that is not accessible to the public.  If there are pictures of me that I want removed, I may ask and have this immediately done by the Administrator of Twin Flame Creations Inc.
