    GENERAL REGISTRATION FORM

  • Please complete the registration form below and submit it electronically, no later than Friday, September 2, 2022. Registration fees are $60 per dancer or $80 per family and are due upon registration. All registration fees can be paid via CashApp ($spotlightstudio), Venmo (@spotlightstudioofdance), or JackRabbit and are due no later than September 2, 2022.

    September Tuition will be billed in JackRabbit and the remaining months will be paid through SmartTuition.  All SmartTuition accounts must be updated and active by Wednesday, August 31, 2022.

    September Tuition payment is due by September 9, 2022, classes begin September 10, 2022.

     Please note for Recital classes, a recital Fee of $85.00 per dancer and a costume fee of $85.00 per recital dance number will be charged in January 2023.  Tuition fees are as follows: 

    • 1 hour per week:        $90.00/month
    • 1.5 hours per week:   $137.50/month
    • 2 hours per week:      $175.00/month

    • If I or the designated alternate cannot be reached in case of an emergency, I give permission to Spotlight Studio of Dance staff to secure emergency medical treatment.

    • Agreements, Waivers, and Signature  

    • By signing below and submitting this form, you, the Parent or Guardian of the above student(s), agree to release Spotlight Studio of Dance, including instructors and assistants from liability for any and all injuries that may occur while training, practicing, performing, or during any studio event or activity. You also agree that you are responsible for health and accident insurance and any medical costs incurred due to injury. You give permission for emergency medical transportation and treatment of your student(s) at your expense should the need arise. You also give your permission for the public display of any studio visual images that your child may appear in.

      In addition, you also certify that you are the parent or legal guardian of the student(s) and have the legal authority to make the representations and grant the authorizations contained herein. You agree to authorize Spotlight Studio of Dance personnel to provide instruction for my child. You also understand and agree to the financial obligations and enrollment terms established by Spotlight Studio of Dance. You understand that you will incur a $25 returned check fee for all returned checks and credit card chargebacks and all payments are non-refundable. NO REFUNDS!
