  • This is the leotard order form for your team for the River City Classic.

     

    PAYMENT INFORMATION

    All Level 6 - 10 & Xcel Platinum and Diamond gymnasts receive this leotard as their gift. Additional leotards are $40 each. 

    Once you have completed this form, add up your total and send a check made out to Premier Gymnastics Booster Club to: Attn: Kelly Bodner, Premier Gymnastics, 10702 Browne St., Omaha, NE, 68134.

    please list each gymnast under each level with their size. PLEASE FIRST AND LAST NAME of the gymnast. The form is user-edited, click 'Add Another' to up to the amount of gymnast you have in each level. This will help with ordering, purchasing and billing.
