PTA Birthday Book Form

Each classroom in our school has its own library and we would love to have the opportunity to grow our classroom libraries. With a $36 gift to the OCA PTA, a new library book or sefer will be dedicated to your child or grandchild's classroom in their honor. A book plate with the child’s name and birthday, along with the name of the sponsor will be placed inside the front cover.

Students will also receive a coupon for a free cupcake from Pariser's Bakery in addition to a birthday card indicating your generous gift. Please sign up for this amazing opportunity below.
