  • In the case of accident or serious illness, I request that the Georgian Triangle Humane Society contact me. If the Georgian Triangle Humane Society is unable to reach me, I authorize the Georgian Triangle Humane Society to make whatever arrangements deemed necessary, such as administration of an Epinephrine autoinjector.

    Attendance Policy: The Humane Education Programs award certificates of completion to each registrant. Please note that certificates will not be granted to anyone who does not complete at least 90% of the program. Refund Policy: Please note that refunds will be available prior to the first day of programming. Payment is for the full eight-week session and therefore refunds for missed days will not be available. If a registrant must withdraw from the program before the end date, refunds may be given for special circumstances.

    Participants' Code of Conduct:

    The safety of both the participants of this program and the animals in our care is of utmost importance to the Georgian Triangle Humane Society. Participation in this program means spending time in a busy animal shelter and interacting with some shelter animals. Participants must adhere to the shelter's dress code which includes: long pants, closed toe shoes and a Tshirt or long sleeved top. It is expected that all participants will be respectful of the public, staff, volunteers and animals at the Georgian Triangle Humane Society and will follow the rules established by the Georgian Triangle Humane Society staff. Any behavior of a participant that places him/herself, or any other human or animal, at risk may result in the participant's immediate dismissal from the Afterschool Programs. If dismissed from the program, the parent/guardian of the participant must cover any expenses arising from such dismissal.

  • Waiver and Release Form for Minors

    Health and Safety Waiver

    I understand that as part of the Afterschool Programs, my child will be interacting with Georgian Triangle Humane Society animals with adult supervision.  I understand that, while all necessary precautions will be taken to avoid harm, there is a risk that my child may be scratched, bitten or come into contact with zoonotic disease as a result of participation in this program.  I assume that risk.

    I hereby agree to release, indemnify and hold harmless the Georgian Triangle Humane Society and its officers, employees or agents from any and all liability for any damage or injury arising from participation in this program.  This release shall include, but is not limited to:  accidents, illness, injuries, or damage to possessions.

    Media Release Form

    I grant permission to the Georgian Triangle Humane Society to use my child’s image (photographs and/or video and/or sound) for use in Georgian Triangle Humane Society publications including:  videos, email blasts, recruiting brochures, newsletters and magazines, and to use my child’s image in electronic versions of the same publications or on the Georgian Triangle Humane Society website or other electronic forms of media and social media, like Twitter and Instagram.

    I hereby waive any right to inspect or approve the finished photographs or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the image.
