Service Form
Filling out this form should only take a few minutes. Providing additional details can help speed up the repair process. Please submit one appliance per form. If you have multiple appliances requiring service, submit a separate request for each. If the property is tenanted, you’ll have the option to include tenant details so we can coordinate the appointment directly with them.
Our minimum call-out and diagnosis fee covers the technician's time, insurance, fuel, dispatcher fees, office fees, training fees, etc. This fee also encompasses the time spent diagnosing the appliance. Repair labor is billed additionally.

Diagnostic fee will be charged at time of booking
Repair labor is billed in addition to call out/diagnostic fee + parts
If a second person is required additional charges are applicable
The technician will call me the evening prior, to provide a 2 hour arrival window
If the technician isn’t able to repair the appliance on the first visit, I will be sent a repair quote for approval
Completed repair will be charged to card on file. (Non-acct customers)
Each appliance serviced has a diagnostic fee
