I would like to stand for election to the AHA Board of Directors at the August 20, 2023 Annual General Meeting.

I understand that this form is required to be completed and returned to the AHA, either by clicking the SUBMIT button at the bottom of the form, or submitting via Mail to P.O. Box 44071 RPO Southcentre, Calgary, AB T2J 7C5, or by email to: info@albertaherbalists.com no later than August 03, 2023 - 15 days in advance of the Annual General Meeting.

This nomination form is required to be signed by three (3) members in good standing. If you do not have signatures, your form will be considered for signing by the board of directors.

A member in good standing is defined as having a fully paid membership in the AHA. All nominees shall have held a full membership for at least one (1) year prior to the Annual General Meeting.
I would like to stand for election as Board of Directors as: (ie. secretary, registrar, vice president, member at large, etc)
and I hereby consent to serve on the AHA Board of Directors if elected.
