I would like to stand for election to the AHA Board of Directors at the August 20, 2023 Annual General Meeting.

I understand that this form is required to be completed and returned to the AHA, either by clicking the SUBMIT button at the bottom of the form, or submitting via Mail to 1749 Turvey Bend NW, Edmonton, AB T6R 2W6, or by email to: info@albertaherbalists.com no later than September 21, 2026 - 15 days in advance of the Annual General Meeting.

This nomination form is required to be signed by three (3) members in good standing. If you do not have signatures, your form will be considered for signing by the board of directors.

A member in good standing is defined as having a fully paid membership in the AHA. All nominees shall have held a full membership for at least one (1) year prior to the Annual General Meeting.
