By submitting this form, I acknowledge and agree to the following terms and conditions on behalf of myself and all registered participants in my group, including but not limited to other adults and the minor(s) under my guardianship:

(1) Medical Treatment: I/We agree to release and forever discharge Nature Collective from any claim whatsoever which arises on account of any incidents, accidents, first aide, treatment, or service rendered to me/us in connection to participation in the above described event. (2) Assumption of Risk: I/We assume the responsibility of mental and physical fitness of myself/ourselves to participate in the scope and activities described for the above described event. In addition, I/We assume the responsibility for any damage to personal property and/or equipment during the transportation and participation in the above described event. (3) Release of Liability: I/We acknowledge that participation in the above described event involves risk and liability, both known and unknown, including but not limited to physical injury (up-to and including death), emotional injury, damage to personal property and/or equipment, and other damages not listed. I/We acknowledge that we are voluntarily engaging in the above described event with full knowledge of these above-mentioned risks. I/We assume any and all risk of injury or damage that may result from the participation in the above described event.

Al enviar este formulario, reconozco y acepto los siguientes términos y condiciones en mi nombre y en el de todos los participantes registrados en mi grupo, incluidos, entre otros, otros adultos y los menores bajo mi tutela:

(1) Tratamiento médico: Yo/nosotros acordamos liberar y eximir para siempre a Nature Collective de cualquier reclamo que surja debido a cualquier incidente, accidente, primeros auxilios, tratamiento o servicio prestado a mí/nosotros en relación con la participación en el evento descrito anteriormente. (2) Asunción de riesgo: Yo/nosotros asumimos la responsabilidad de mi aptitud mental y física para participar en el alcance y las actividades descritas para el evento descrito anteriormente. Además, yo/nosotros asumimos la responsabilidad por cualquier daño a la propiedad personal y/o equipo durante el transporte y participación en el evento descrito anteriormente. (3) Liberación de responsabilidad: Yo/Nosotros reconocemos que la participación en el evento descrito anteriormente implica riesgo y responsabilidad, tanto conocidos como desconocidos, que incluyen, entre otros, lesiones físicas (hasta e incluyendo la muerte), lesiones emocionales, daños a propiedad y/o equipo, y otros daños no enumerados. Yo/nosotros reconocemos que participamos voluntariamente en el evento descrito anteriormente con pleno conocimiento de estos riesgos mencionados anteriormente. Yo/nosotros asumimos todos y cada uno de los riesgos de lesiones o daños que puedan resultar de la participación en el evento descrito anteriormente.
