Monthly Membership Contract

By completing this form, you are signing up for the monthly membership program with Bronzed Babe Airbrush Tanning. Membership will begin on the 1st day of the month following contract and invoice completion. All memberships are billed on a monthly basis for a six month commitment. Upon completion of the Monthly Membership Contract a separate invoice MUST be completed to authorize monthly billing. Membership will not take effect until invoice is completed and authorized for a recurring monthly charge.

Policies

All memberships are subject to a six month minimum, early termination fees apply. Monthly membership tans are non transferrable and non refundable. Tans must be used by the end of each calendar month, unused membership tans do not rollover. All appointments are subject to availability and are not guaranteed by Bronzed Babe Airbrush Tanning.

The monthly memerbship cost will be billed on the first of every month to the card added on file. It is the responsibility of the client to inform Bronzed Babe Airbrush Tanning if any changes or updates have been made to their card on file. Any payment interruptions are subject to a 5% inconvenience fee or early termination fee.

Upon completion of the Monthly Membership Contract an email will be sent out within 24 hours containing the Membership Invoice. Inovice must be completed within 24 hours by the 1st of the month to begin monthly recurring charges.

Cancellations

All memberships are subject to a six month minimum. Early termination of a monthly membership will result in an early termination fee equal to 50% of the remaining membership respnsibility. Any cancellations after the six month minimum must be made in writing and emailed to bbtanning831@yahoo.com 48 hours prior to the next billing cycle. Any cancellations made within the 48 hour cancellation window will be subject to a 50% late cancellation fee. Any cancelled memberships are final and can not be reinstated for a six month period.

I authorize the company to regularly charge my card on the amount indicated in this form on a regular basis, and shall remain in effect until I request for the cancellation or termination. I likewise certify that I am the authorized user of the credit card that shall be submitted through this form. As long as the transactions correspond to the terms and conditions indicated in this authorization, I shall not raise disputes against the company. I authorize that I am 18 years of age or older and able to enter and sign this contract.
