Order Form #2: Trees for Sylvan Heights, March 2023
For residents of the Sylvan Heights neighborhood, with planting captain Kelly Nagy: Completing this form confirms your tree order. After completion, you will receive a packet of information delivered to your front porch 3-6 weeks before your trees arrive. You will also receive email reminders with instructions for how to prepare your tree. Please review your order details below and make changes as needed. You can use this link to edit your order or update your delivery information until the week prior to delivery.**PLEASE NOTE: all trees have already been sourced from the nurseries, so we cannot approve increases to your order or swapping of species at this time. IF YOU NEED TO CANCEL OR DECREASE YOUR ORDER - please adjust your total tree quantity AND your selected species to "0" or the decreased number. Our crew works hard to deliver trees in a timely manner straight to your yard, next to their holes for easy planting. You can help us ensure another successful delivery season by planning ahead, keeping your information current, and entering your delivery details below. Happy planting!

I understand that my tree will be about 6-8 feet in height at the time of delivery, and that the hole for each tree needs to be 3-4 feet wide and 1.5 feet deep.
*
Yes, I can handle this!
Last minute cancellations due to a lack of hole digging is our biggest challenge to keeping the trees free and available. Please think ahead now! Who will dig your holes PRIOR to the Monday of your delivery week? What is your backup plan if this falls through? (If you were approved for volunteer help before the deadline, you can note that here.) Thank you for your careful consideration of this question - see you soon!
