DVR Course Registration Form
Conditions:
- Cancellation for courses will only be allowed 48hrs before the course commences.
- Anyone who booked for the courses who do not turn up or who do not cancel the course 48 hours before the course will be charged the full rate of the course as listed below.
- The courses are not catered events; please arrange your own meals. Only tea/coffee and biscuits/rusks will be served.
- Application forms that have not been fully completed and signed by the applicant will not be considered.
