Based on your responses, the option below seems like the best fit for your situation

Renewal request: ALL current roommates want to renew the lease for another term

Roommate change request: SOME current roommates (but NOT all current roommates) want to be removed from the lease AND/OR you want to add a new roommate for the current or future lease term

Sublet request: ONE OR MORE current roommates want to be replaced on the lease by a different roommate for a limited amount of time during the current lease term

Notice to Vacate: ALL roommates do NOT want to renew but plan to move out at the Lease End Date

Notice of Lease Break: all current roommates want to leave the lease before the end of the current lease term

Some cities (such as Burlington and South Burlington) charge an annual rental registry fee, and tenants will be required to pre-pay this fee during the renewal process

I understand and acknowledge

2 months (60 days) advance notice is required for Lease Breaks. However, the 60 day countdown of this notice starts on the month following notification and payment of the Lease Break Fee. Therefore, the earliest possible move-out date that will be accepted for Lease Breaks is 2 full months after the start of the next, upcoming month (provided that you have completed this form and paid the Lease Break Fee).

For Lease Breaks, we recommend (but it is NOT required) that you select a move-out date that is 10 business days before the end of the month to ensure that Full Circle Property Management is able to complete any necessary repairs or cleaning that are necessary for leasing to a new tenant. Selecting a date that is closer to the end of a month may delay the leasing process.

Any requests to change the lease by adding a roommate, replacing a roommate, or a a sublet requires prepayment of a Lease Change Request Fee. This fee covers the costs associated with processing your request. This fee is non-refundable and does NOT guarantee approval of any potential new roommate. This is a one-time fee, so if a particular applicant is not approved and you need to identify a different applicant, then you will NOT need to pay any additional Lease Change Fee.
