Some students in our Grace School community use Augmentative and Alternative Communication (AAC). This includes devices, systems, strategies and tools that replace or support natural speech when spoken words may not be easily understood by others.
As it is natural for students to use the names of their peers in everyday communications, AAC can be programmed to allow users to call on or reference their peers by name.
By offering your consent, you give permission to allow Grace School students' AAC to store and use:
a) an identifying photo/image of your student and
b) your student's name.
Owned by individual students/families, AAC is not the property of The Grace School. Any personal health information or other information stored on AAC may be subject to re-disclosure and may no longer be protected by applicable Federal and State privacy laws.
If you consent, then authorization is valid from the date of your signature. You have the right to revoke this authorization by providing written notice to The Grace School. However, this authorization may not be revoked if The Grace School, it’s employees or agents have taken action on this authorization prior to receiving my written notice.
This authorization is voluntary and may be refused. Your refusal will not affect eligibility for benefits, enrollment in educational services, provisions for therapy care, or payments.
