* This field is required.By signing below I acknowledge that hair color involves processes using chemicals. I understand that my stylist will do everything in her power to achieve my goals while preserving the health of my hair but also acknowledge that there are many factors and variables that can influence how the chemicals react to my hair that is often times out of my stylist’s control. I understand that if I fail to notify my stylist of box dye or non-professional color a $100 fee may be added onto my service. If there is any problem with my color, I will notify my stylist within 7 days for an adjustment free of charge; if notified later than 7 days after the appointment, the adjustment will be full price. I understand that if for any reason I need to cancel my appointment, I must give my stylist 48hrs notice so that she has time to fill that spot. Notice given less than 48hrs before the appointment will result in a cancellation fee of 50% of the service total. Same day cancellations and No-call, no-shows will result in a flat rate charge of 100% of the service cost. If you are more than 15 minutes late to your appointment, you are subject to a $35 late fee if your stylist is still able to get you in without disrupting her schedule. If you are too late for her to get you in, you will be subject to a fee equal to 50% of the service and you will need to reschedule. If you are more than 15 minutes late and don’t give your stylist notice that is considered a no-call, no-show and the associated fee will be applied. I consent to my photo being taken and acknowledge that any photos taken are Coastal Hair Co. property and may be used for marketing purposes. Anddddd now after all that, I ACKNOWLEDGE THAT I AM SO EXCITED TO GET MY DREAM HAIR AND FEEL LIKE MY MOST BEAUTIFUL SELF. WAHOO!!! So so excited for you babe!
