  • This form should be returned completed, after signature, to the Club Manager on or before the first booking date along with a copy of your risk assessment. The booking is only confirmed upon receipt of a confirmation e-mail from the Club Manager or a member of staff authorise to act on the Club Managers behalf and guaranteed by payment in advance of the first date.

  • Agreement is approved subject to the conditions and charges of the School Governors for the use of the:

  • The initial month’s bookings for new block bookings must be paid at the time of booking. Invoices for Block Bookings will then be issued on a monthly basis in arrears and must be paid within 14 days. Any future bookings may be cancelled if an invoice remains outstanding after this time. Bookings of less that 10 sessions or “Pay as you Go” bookings must be paid in advance. Bookings are only confirmed on receipt of a confirmation e-mail from the Club Manager and on receipt of initial payment.

    I being the duly authorised agent of the named organisation above, agree to abide by the conditions and regulations enclosed and do hereby agree to indemnify 3d Health & Fitness against any liability whatsoever which may arise out of the letting of the premises to me.
