    Below reports are enhanced features we offer to automate. If you would like these to be turned on, please let us know. 

  • You will need to add this message template with the title as mentioend above to utilise prediabetes module. Please ensure patients can reply to the message by default and SNOMED code is enteredas mentioned in the guide. 

  • You can choose your custom message content however, please ensure the title of the template must match "NDH (Prediabetes) offer to refer to NDPP". You also need to check the box if you wish patients to reply. 

  • We have defaulted a "Good Diabetic Control" as an HbA1c result between 48-54 mmol/mol.

    We recognise you may wish to align this with QOF and as a result, please indicate below whether you would like to adjust the upper and/or lower limit.

  • We are able to exclude specific HbA1c reports if required for example patient's on insulin. Please indicate if you would like any specific exclusions.

    Software licence clause

    7.2 Where the Software takes (or omits to take) certain actions (including but not limited to sending text messages to patients) based on test results falling in certain agreed ranges, the Customer acknowledges and agrees that:

    (a) the Software is not making decisions or applying automated reasoning but is merely automating unskilled tasks normally carried out by the Customer’s own employees;

    (b) the Software can be configured to disable those actions and the Supplier will only enable those actions if the Customer gives its explicit consent in writing;

    (c) if the Customer consents to those actions being enabled it takes full clinical responsibility for the outcomes of those actions; and

    (d) consequently, in accordance with the guidance provided by the Medicines and Healthcare products Regulatory Authority, the Software is not a medical device.
