APA Division Champs Award Selection Form

This Request Form is only for Team Captains of Division Championship Teams to select awards for their teams at the end of the session. This selection is all or none. There will not be any combination of shirts, trophies, and cash allowed. If a team cannot agree, the default selection will be Trohpies.

All teams will receive a Team Plaque and Patches regardless of award selection.

Please note that the name on the plates will be exactly as it is on the scoresheet with nicknames included unless otherwise specified.

Teams must be up to date with all League payments to receive the award.

Additionally, as the Team Captain you are attesting that the selection made below was decided upon by your team by a majority vote.
