Informed Consent and Acknowledgement

I hereby give my approval for my child’s participation in any and all activities prepared by Templeton Presbyterian Church for the 2022-2023 Ministry Season. I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless Templeton Presbyterian Church and all its respective staff, volunteers, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from any activities.

In case of injury to said child, I hereby waive all claims against Templeton Presbyterian Church including all Staff, Volunteers and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all water based activities, including swimming. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.

In addition, Templeton Presbyterian Church cannot prevent parents, students, leaders, staff or guests from becoming exposed to, contracting, or spreading COVID-19 while utilizing Templeton Presbyterian Church’s services or premises. It is not possible to fully prevent against the presence of the disease. Therefore, if you choose to utilize Templeton Presbyterian Church’s programs and/or enter onto Templeton Presbyterian Church’s premises you may be exposing yourself to and/or increasing your risk of contracting or spreading COVID-19. The undersigned further hereby assumes all risk posed by the novel Coronavirus, COVID-19.

The undersigned further hereby agree to hold harmless and indemnify said church, its directors, employees and agents, for any liability sustained by said church as the result of the negligent, willful or intentional acts of said participant, including expenses incurred attendant thereto.                                                        

If the participant has not attained the age of 18 years:

We (I) are the parent(s) or legal guardian(s) of this participant, and hereby grant our (my) permission for him/her to participate fully in said trip, and hereby give our (my) permission to take said participant to a doctor or hospital and hereby authorize medical treatment, including but not in limitation to emergency surgery or medical treatment, and assume the responsibility of all medical bills, if any.       

Further, should it be necessary for the participant to return home due to medical reasons, disciplinary action or otherwise, we (I) hereby assume all transportation costs.
