As the applicant/customer, I understand that:
1. Water and sewer service is charged on a flat rate and that fees are subject to change annually.
2. A late fee is charged to the account for all unpaid balances remaining on the first day of each month.
3. A customer (and the property owner, if different from customer) will receive a Late Notice by mail for all unpaid balances that are more than 30 days past due. If a customer does not respond to by phone, email, or in-person at City Hall to the Late Notice or a payment is not received, a 10-Day Shut-Off Notice will be placed on the door of the residence. Whether you actually receive this notice or not, placement of the 10-Day Shut-Off Notice is considered to be legal notice that if the unpaid balance on your account is not paid within the next ten days or you do not contact City Hall to make payment arrangements, your service will be terminated for non-payment.
4. If your service is terminated, you will be required to pay all amounts due, including fees determined by Resolution No. 06222022-01, before your service will be reconnected.
5. Failure to receive your bill or a delinquent notice or a door hanger does not remove your responsibility to pay any amounts due as required and will not allow you to avoid scheduled terminations for non-payment. Other formal processes may be used if bills become delinquent.
6. The customer and/or property owner is responsible for notifying City Hall in writing when a new customer and/or property owner will be moving or transferring any services. The City of Maupin will pro-rate service based on the move-out date.
