Extensions are a luxury services and it is my obligation to provide the best quality products and services to my guests. I encourage you to do some research on GreatLengths. I am confident you will understand why I choose GreatLengths as a brand.

Once I receive this form we can set up a consultation. During that time we will go over, color, length, pricing, aftercare and any questions you may have. A $75 fee is due the day of your consultation. If you choose to go through will the extension service a 50% non-refundable deposit is due to hold your spot and your consultation fee will be waved.

CANCELLATION POLICY Out of courtesy to wyour stylist, I kindly ask that you give 48 hours' notice for any cancellations or reschedules. In the case that more than 48 hours' notice is not given for a cancellation or reschedule, you will be charged a $100 non refundable cancellation fee.
Please be aware that if you are more than 15 minutes late for your appointment, I may not be able to provide the service for which you have been scheduled. As a courtesy to other guests, please notify me directly if you are at all running late (even 5 minutes makes a difference).
