SWWS Membership Application Form

I wish to become a member of Sarawak Women for Women Society (SWWS). I will contribute & support SWWS to achieve its aims and mission in a feminist spirit. I understand that as a member, I will have voting rights in SWWS's Annual General Meeting. I understand that my membership application is subject to confirmation by SWWS' Committee. I commit to SWWS's Core Values and SWWS's Constitution.

Please note that annual membership fee (RM 20) is due by 31st December. A membership shall be considered as lapsed if the member does not renew the membership for 2 years.

Upon payment, please text 013-804 4285 (SWWS) or email info@sarswws.org with your name and proof of payment for us to issue a receipt and update our records.

You will only be required to pay the membership fee upon communication of acceptance of your application.

The personal information you provide to Sarawak Women for Women Society ('SWWS"), whether now or in the future, may be used, recorded, stored, disclosed, or otherwise processed by or on behalf of SWWS for the purposes of facilitation & organisation of your involvement with SWWS; events; research & audit & maintenance of a membership data base, and such ancillary services as may be relevant.

Any information that you learn about, or have access to regarding, Sarawak Women for Women Society, including its clients*, employees, members, volunteers, donors and any affiliated parties, that is not publicly available, constitutes "Confidential Information".

You are required to undertake and agree to keep any Confidential Information strictly private and confidential, and shall not directly or indirectly disclose, publish, transfer, disseminate, copy or permit to be disclosed, by any means, such Confidential Information to any unauthorised personnel or third parties. This provision shall extend even beyond your affiliation with Sarawak Women for Women Society.

Sarawak Women for Women Society reserves the right to take legal action in the case of any breach of this condition, even if you do not actually benefit from the disclosure of such Confidential Information.

*Clients refers to the women and children, including their family members, who seek SWWS's assistance.

Membership Application Confirmation

By signing this form you have consented to receive information about SWWS via email/mail and to go through the membership application process. You will only be required to pay the membership fee upon communication of acceptance of your application.
