The UPOU Office of Student Affairs (OSA) is mandated to provide information, advice, and guidance regarding the students’ psychosocial, financial (scholarships and student loan), personal/ emotional, and assistance in your academic concerns. We have responded to some students’ issues amid the COVID-19 crisis through the O.S.A. Helpdesk. We would like to extend our assistance in dealing with matters that may be helpful to you specifically about your psychosocial concerns.
You will be asked about your personal information and contact details, and the specific details about the inquiry and/ or concern that needs attention. By filling out this form, you authorize OSA Psychosocial Online Support Hub to collect and retain personal and contact information, including but not limited to your name, email address, telephone number, and other details, for the purpose of recording and providing you with the said assistance. When needed, you will receive another consent form on the use of technology for psychosocial support and consultation (CGP-OSA Form 2).
Any data submitted through this form will be treated with utmost confidentiality and will be used solely for assisting you in your individual inquiries and concerns.
