As a community member (not a UChicago student or employee), you are required to provide proof that you have received at least two doses of a COVID-19 vaccine before attending any rehearsal or event on campus related to the Performance Program. You can find vaccination options near you at vaccines.gov.

Booster shots beyond the initial two vaccine doses will not be a University requirement; however, the University strongly recommends that individuals continue to receive vaccine boosters as recommended by the Centers for Disease Control and Prevention.

You must submit proof of vaccination via the file upload feature below. If you have not yet received two doese of a vaccine, but will be before rehearsals begin, please submit a document that includes dates and location of intended vaccine doses.
