Paige does NOT tattoo areas including: ribs/side, stomach, behind the ear, back of the neck, side of fingers, chest/collarbone, butt, or anywhere below the ankle. She only offers fonts posted on her instagram and no longer accepts Roman numerals. She also does NOT add onto existing work from other artists, or do correction work

Yes, I understand and I understand if any of these are in my form that my form will be looked over.
Paige does NOT allow any type of numbing cream to be used during her tattoo sessions- check her “Ink Academy” highlight for more information on this topic!

I understand that Paige does NOT allow numbing cream and I agree to NOT use any for my appointment. If i do so I will be asked to reschedule, and I will have to pay 75% of my sessions total that day to compensate Paige for my wasted appointment time.
Once your request is approved- upon scheduling a $50 NON- REFUNDABLE deposit is required per appointment scheduled. This is a NON-NEGOTIABLE POLICY. (This can be found on my site under Policies)

I acknowledge and accept Medusa Ink’s DEPOSIT Policy
If you cancel within 24 hours of your appointment time it will result in a forfeited deposit and an additional charge of 75% of the quoted tattoo price. If you cancel within 48 hours of your appointment time it will result in a forfeited deposit and an additional charge of 50% of the quoted tattoo price. No Shows will forfeit their deposit and be charged 100% of the quoted tattoo price. These are NON-NEGOTIABLE policies.
