Animal Surrender/Intake Request

Current waiting times are very long for cats, dogs, rabbits and guinea pigs. This can change quickly and if we don't have a form you will not be on our waiting list. If you find an alternative space please contact us to let us know. Other animals we may be able to help within a week.

When we are able to offer a space we will ask you to sign a sign over form that will release ownership to Berwick Animal Rescue Kennels (BARK) on a permanent basis. It will declare you are the legal owner, or their representative and you relinquish your the animal named on the form to BARK. Please understand that this is a permanent arrangement and BARK will Take unconditional possession of our animal as detailed on the sign over form. Your animal will be assessed, given any necessary veterinary treatment and found a suitable permanent home as soon as possible. It is important that your realise this, and although you are welcome to contact us to find out how they are doing you can not come back to us anytime after you have signed over your animal asking for them to be returned. This will not happen. Please ensure you have explored all avenues before brining your animal in for rehoming. Completion of this Animal Surrender/Intake request is not signing the animals over to BARK.

If you need to rehome your animal, we would ask you to fill out the attached as much as possible. If you are asking us to take multiple animals please complete form for one and put details of other animal in additional notes. Photographs are helpful but not essential.

Please be as truthful as possible, nothing you say will change when we will take your animal but it will give us and them the best chance of finding a new home as quickly as possible.
