Preferred Training Campuses Brown Academy of Sports will be conducting classes at 2 different middle school campuses throughout the district.To help us make coaching assignments, please list your top campus preferences. While we cannot guarantee placement at your preferred locations, we will do our best to accommodate your requests whenever possible.

Photo & Media ReleaseI give Brown Academy of Sports permission to photograph or record my child(ren) during program activities for promotional, educational, and social media purposes.

Parent Agreement & Liability Waiver

I acknowledge that participation in athletic training, strength and conditioning, agility drills, fitness activities, and related programs involves inherent risks that may result in injury. I understand these risks and voluntarily allow my child(ren) to participate.

I certify that my child(ren) is physically able to participate and that I will notify Brown Academy of Sports of any medical conditions that could affect participation.

I agree to release and hold harmless Brown Academy of Sports, its owners, employees, coaches, volunteers, facility partners, and affiliates from any and all liability, claims, demands, or causes of action arising from participation in the program, except in cases of gross negligence or willful misconduct.

I understand that Brown Academy of Sports reserves the right to remove any participant whose behavior is unsafe, disruptive, or inappropriate.
