Please follow the instructions in the Lifeguard Management guide found on the NCSL website to complete the following form. Please complete one form per lifeguard per payroll period.
Forms are due the day after the pay period ends.
If you need to edit your form after submission, a link will be provided in your confirmation email. DO NOT create a duplicate form for the same pay period.
