    This form is part of the enrolment process for the Yorkshire Hypnotherapy Training Practitioner Course.

    Completing it helps me get to know a little more about you and any support you might need during your studies, so that I can help you get the best possible start to your training.

    Once I've received your form, unless we've already had one, we'll arrange a short, informal chat. This is an opportunity to make sure:

    • all your questions have been answered,
    • you have a clear picture of what the course involves,
    • we have discussed any practical arrangements or support needs.

    If a place is offered, a deposit and signed learning contract will secure your place on the course.

     

    Accessibility

    Our training room and toilet are located upstairs in a Victorian building and unfortunately there is no lift. If this raises any access needs or concerns, please feel free to get in touch first, so we can talk things through.

    Some unspent criminal convictions may affect your ability to obtain professional registration, insurance, enhanced DBS checks, or employment opportunities after qualification.

    To help applicants make informed decisions before enrolling, I ask the following question.

  • ARRANGING A CHAT

    When you submit this form, you’ll be directed to my booking calendar. I like to have a quick chat with everyone before confirming their place on the course. Nothing formal, just a chance to get to know you a little and hear what’s bringing you to the training.

    If you have already had a Discovery call, you don't need to make another appointment. 

  • By submitting this form you are accepting that

    • places are offered on the basis of the information you supply and you affirm that this is true and complete
    • classes can only run if student numbers are viable
    • for your protection, and that of others, we reserve the right to ask for confirmation from a medical practitioner that it's safe for you to take part in the course and/or practice after qualification, if you have a long-term or serious mental health condition
    • we reserve the right not to offer you a place if classes fill or if we feel you are not a good fit for our course
