Pre-Authorized Debit (PAD) Details

I, the Payor, authorize York Swim Club to make pre-authorized debits (PAD) from my account (as described above).  Each withdrawal will be a variable amount for which I must be advised by York Swim club in writing (email) at least 10 days before the debit date for York Swim Club services (program fees, registration fees, meet fees, and other club membership charges).

I acknowledge that NSF fees will be my responsibility if payment is declined for any reason.

I will notify York Swim Club promptly in writing if I move the account from one bank or branch to another, or if there is any other changes in the account.

I, the Payor, may revoke my authorization at any time by emailing York Swim Club (yorkswimclub@gmail.com) subject to providing notice of at least 15 days. You have certain recourse rights if any debit does not comply with this agreement. For example, you have the right to receive reimbursement for any debit that is not authorized or is not consistent with this PAD Agreement.
