  • Competition starts Tuesday 30th September 2025

    Team Nominations and payments are due by Tuesday 23rd September 2025. If you have not paid by this date your team nomination will not be accepted.

    There will be 2 sign on events at MUFC Clubhouse:

    1. Tuesday, 16th September 2025 at 6:30pm - 8:30pm

    2. Tuesday, 23rd September 2025 at 6:30pm - 8:30pm

    TEAM NOMINATION FEES:  $770.00 PER TEAM (min 6 - max 10 players)

     **only one payment per team will be accepted**

  • Team Insurance Waiver and Release of Liability

    As Team Captain, by submitting this form I confirm that:

    • Risk Acknowledgement
      I understand that football involves physical contact and activity that carries a risk of injury. All players on my team accept this risk voluntarily.
    • Waiver and Release
      On behalf of myself and all players on my team, I release and discharge Mareeba United Football Club, its officials, volunteers, sponsors, and affiliates from any liability, claims, or damages arising from participation in this competition
    • Insurance Responsibility
      I acknowledge that Mareeba United Football Club does not provide personal injury or medical insurance. All players are responsible for their own coverage.
    • Medical Consent
      In the event of injury or medical emergency during the competition, I authorise the Club to seek necessary treatment for any of my team members. All medical expenses will be our responsibility.
    • Team Responsibility
      I confirm that all players have been informed of this waiver and agree to its terms. I accept full responsibility for my team's compliance with the Club’s rules and code of conduct.

    As team captain it is YOUR responsibility to show this waiver to your team members and to seek approval from them before finalising this nomination form.
