UNDERSTAND THE FOLLOWING: The eyelash extension procedure requires synthetic eyelashes to be glued to my own natural eyelashes. It is my responsibility to keep my eyes closed and be still during the entire procedure until the technician leads me to do the opposite. There are some risks of this procedure such as eye redness and irritation. The fumes from the adhesive may cause my eyes to tear up and become irritated if I do not keep my eyes closed. I disclosed any allergies that I may have. I understand that contact lenses are advised to not be worn during services. Glasses should be worn if applicable. I agree to the aftercare instructions given and realize that the failure to follow the instructions may result in rapid lash shedding and an overall decreased life of my extensions (oils, alcohol, not washing, getting wet too soon, etc.) Key To Glam is not responsible for failure to follow aftercare instructions and/or poor maintenance. By signing this form, I am acknowledging and understand the terms of the service as well as the information listed above, this agreement will remain effective for the duration of the service, and any proceeding eyelash extension services in the future. I consent to this agreement and services.

A nonrefundable $15 Venmo deposit is required to secure your appointment. PLEASE only put date and time in the transaction note (ex. “5/24 at 3:30pm”). Your deposit comes out of your total service cost. I understand that if I am 15+ minutes late and haven’t spoken to Abby, that my appointment will be canceled and I can reschedule for an appointment at a later availability. Missing an appointment will result in the need of a new deposit. I understand that deposits are advised to be sent upon booking, but MUST be sent 24hrs prior to appointment. I understand that if my deposit is sent less than 24hrs prior to my appointment, it will be considered a “squeeze-in” appointment and I will be charged an additional $15 fee upon checkout. Appointments will not be confirmed until deposit is received.
