Bay Area Community Church Short Term Missions
Dear Future Short Term Missionary:

Welcome to the GO Form for short term missions (STMs) at Bay Area Community Church!

This is your first step to partnering with Bay Area in 2023 to share the love of Jesus with people all over the world. Please set aside about 20 minutes to fill out this form. Through this document, we will get to know you and your team better, allowing your team leader to build a cohesive, well-organized team. Please answer honestly and fully.

Joining your team isn't only a multi-day trip - it is a months-long adventure! STMs start three to six months before your trip and end about three months after. Your team will meet 6-8 times prior to travel to prepare and will also meet to debrief your experience once you return home. Through these meetings, you'll get to know each other and build practical skills to serve together on the missions field.

What happens next?

This form has two parts - this first step collects information about your spiritual journey and missions experience, while the second step collects important travel information and a $50 STM registration fee. You can return to this page to access part two at any time:

Passport Information

In the final section of this GO Form, you will need to provide your passport information if you are traveling outside of the US. If you do not have a current US passport and have not started the process to get one, OR if your current passport will expire less than 6 months after your proposed trip dates, go to the following link to begin the process: http://travel.state.gov/passport/. For now, please tell us if you are traveling internationally or within the US, and whether or not you have a valid passport.

Additionally, adults traveling after May 3, 2023 will be required to have a REAL ID compliant drivers license. https://www.dhs.gov/real-id for more information.

Background Check Authorization

Short Term Missions participants over 18 years of age must have a background check on file with Bay Area due to the possibility or likelihood of interacting with children. By checking "yes" below, you authorize Bay Area to complete a background check if you do not have a current one on file at the church. A current background check is required to travel on a Bay Area short term missions team.

You said you were under the age of 18, so we require some additional information from you.

Each of your parents or legal gaurdian(s) who is not traveling with you will be required to send a notarized letter giving permission for you to travel. Letters must be received no less than six (6) weeks prior to travel. Please ask your team leader or e-mail alissa.brandon@bayareacc.org for more information.
